DIALYSIS NURSE (RN) - Chicago, IL (Beverly, IL)

Dialysis Care Center (DCC), a rapidly growing boutique dialysis provider based in the southwest suburbs of Chicago, is seeking a Dialysis Nurse at our Chicago, IL (Beverly) location.

Position Details: The Dialysis Nurse will be responsible for monitoring and directing the day-to-day patient care of Patient Care Technicians (PCT). Serves as a role model and a coach to PCTs. Ensures accurate and complete documentation by PCT on Hemodialysis Treatment Record.

The responsibilities of this position include, but are not limited to:

  • Ability to work independently, and thrive in a highly collaborative, fast pace environment. Ability to multi-task and delegate responsibilities as allowed within scope of practice. Adheres to all facility policies, procedures, and protocols. Participates in staff training and orientation of new staff as assigned. Participates in all required staff meetings.
  • Supervises and performs direct patient care including but not limited to taking and monitoring vital signs, pre- evaluation, initiation, monitoring, termination, access homeostasis, and post evaluation of dialysis, documentation, physical nursing assessment of patients, administering dialysis medications, transcribing physician orders, assessing dialysis adequacy, drive catheter reduction, participation in care conferences and in QAPI, may assist with patient scheduling.
  • Monitoring and reviewing lab results, ordering and following up of labs through Ascend, ensures correct lab tubes are utilized for prescribed lab specimens and that lab draw and processing procedures are performed accurately for all lab samples, ensure that all labs are packaged for shipping correctly, reports alert/ panic and abnormal lab results to physician.
  • Communicates all emergencies, patient life- threatening complications, and related situations to the attention of the medical director, physician, and clinical manager. Maintains competency with all emergency operational procedures. Ensures verification and availability of adequate emergency equipment.
  • Assesses patients response to hemodialysis treatment therapy, making appropriate adjustments and modifications to the treatment plan as prescribed by physician, assessing target weights,
  • Ensuring compliance with safety and infection preventative measures, performing safety and infection audits on a routine basis, ensuring patient safety at all times, monitoring Adverse Event Occurrences.
  • Educating patients and family members on all aspects of End Stage Renal Disease, collaborates with other disciplines to ensure patient outcomes are being met, instructs patients on pre and post dialysis care, provides ongoing continuous education to patients regarding their renal disease, vascular access and dialysis therapy, fluid management, dietary modifications, medication compliance, discuss modality awareness and transplant awareness.
  • Performs additional duties, as assigned.

Requirements:

  • Must have minimum one year of nursing experience, dialysis experience required.
  • Must be PC proficient with experience using Microsoft Office Suite: Outlook, Word and Excel
  • Good presentation & excellent written and oral communication skills
  • Must be productive & resourceful with the ability to meet deadlines
  • Strong data entry, organizational, high integrity, good work ethic, and interpersonal skills

Education and Experience:

  • Must possess RN license in good standing with the State licensing board

JOB DESCRIPTION:

The Home Therapies Dialysis Nurse is responsible for coordinating peritoneal dialysis or home hemodialysis care and includes patient training, primary nursing, and education of home treatment to patients.

A typical day includes drawing labs, training patients on Baxter peritoneal dialysis equipment or the NxStage system within our training centers, running monthly dialysis clinic with physicians, and home visits approximately once a month.

We provide extensive training to our nurses to ensure that they are comfortable and confident in their ability to train and care for patients.

RESPONSIBILITES:

The position will include working with both in-center dialysis patients and home dialysis patients. Our mission is to ensure that patients have the freedom to experience optimal dialysis care whether at an in-center dialysis location or in the comfort of their own homes. The responsibilities of this position include, but are not limited to:

  • Provide nutrition education for dialysis based on modality, patient’s level of knowledge, and individual nutritional and educational needs.
  • Recommend therapeutic diets in consideration of cultural preferences and changes in treatment based on the patient’s nutritional needs in consultation with the patient’s physician.
  • Counseling a patient, a patient’s family, and patient’s significant other on prescribed diets and monitoring adherence and response to diet therapy.
  • Provides ongoing monitoring of subjective and objective data to determine the need for timely intervention and follow-up.
  • Assessment and monitoring of weight changes, blood chemistry, and medication which affect nutrition status and potentially cause adverse nutrient interactions.
  • Complete assessments, participate in care plan meetings, and complete monthly documentation in accordance with facility policy.
  • Participate in IDT meetings including monthly quality meetings and participate in and ongoing effort to improve facility outcomes.
  • Collaborates with IDT on management and recommendations for mineral metabolism, renal bone disease, nutritional status, and sodium and fluid control.
  • Other duties as assigned

Requirements and Experience:

  • A minimum of 1 year experience as a Registered Dietitian in clinical nutrition
  • Dialysis patient experience, strongly preferred
  • Demonstrate effective communication skills, both written and oral
  • Ability to prioritize well and meet overlapping deadlines

Education:

  • Registered Dietitian as per Commission on Dietetic Registration.
