Position Details:

Supports the mission, core values and customer service of Dialysis Care Center (DCC)/Home Dialysis Services LLC (HDS).

Adheres to the Home Dialysis Services Policies and Procedures and Compliance Program including all state and federal regulatory requirements.

The Home Hemodialysis Registered Nurse is responsible for coordinating care related to home modalities including patient training, primary nursing, and education of home treatment to patients.

The responsibilities of this position include, but are not limited to:

  • Provides safe and effective Home Hemodialysis training, direct patient care and delivery of Home Hemodialysis to patients in compliance with standards outlined in the facility policy and procedure manuals, as well as regulations set forth by the state and federal agencies.
  • Rotates with other licensed staff/Home Caregivers to ensure adequate patient care management coverage.

  • Provides ongoing education to patients regarding their home dialysis, Hemodialysis catheter/access and home dialysis therapy, medications, and other related health conditions.
  • Provides on-going education and support to the Home Program Manager in Hemodialysis.
  • Provides on-going support to the Home Dialysis RN/HHD
  • Provides on-going support, education for all Patient Care Technicians/Staff Assistant Caregivers/HHD.
  • Participate in individual & group projects as directed by management
  • Provides safe and effective delivery of home dialysis treatment and support care to Home Dialysis Patients as well as Staff Assisted Home Hemodialysis patients.
  • Assesses patients’ response to home dialysis treatment by following predetermined protocols.
  • Communicates patient related issues to the Director of Nursing/Home Program Coordinator, and the patient’s physician as needed.
  • Reports adverse patient events at home or in the facility/clinic as well as equipment and technology related problems to the Director of Nursing/ Home Program Coordinator and physician, including the correct documentation of such events.
  • Assists with the coordination of a required transfer of the assigned patient to the hemodialysis clinic when appropriate.
  • Reviews, transcribes, and enters physician lab orders accurately into the Medical Information System as required.
  • Maintains appropriate skill level in the operation of all home dialysis therapy-related equipment, including the ability to troubleshoot equipment problems with the patient via telephone.

DIALYSIS TECHNICIAN - LEESBURG, FL

Dialysis Care Center/Home Dialysis Services, a rapidly growing boutique dialysis provider based in the southwest suburbs of Chicago, is seeking a Home Hemodialysis Technician (HHD) at our Leesburg, FL location. There will be on-the-job training.

Position Details: The Home Hemodialysis Technician (HHD) will be responsible in tending to ill and injured individuals under the supervision of doctors, nurses and medical professionals. Their duties include taking vital signs, collecting specimens and assisting patients with the dialysis process.

The responsibilities of this position include, but are not limited to:

  • Monitor vital signs (temperature, pulse etc.) and patient condition, and document observations appropriately
  • Take fluid samples or other specimens for testing, and prepare for shipping
  • Assist nursing staff in administering basic treatments, i.e. hemodialysis therapy, etc.
  • Under the direction of a registered nurse, Monitor and document status of, and instruct patient in the care of, vascular accesses, etc.
  • Review lab work, physician orders, and reinforce treatment care with patient
  • Enter appropriate ICD-9 codes for lab draws into the computer
  • Perform basic nursing procedures (e.g. changing bandages)
  • Ensure patients have adequate patient care supplies and monitor inventories
  • Ready the dialysis equipment before treatment and care for the equipment after treatment
  • Complete flow sheets (via EMR) in order to verify patient billing
  • Comply with company policies and procedures in accordance with state and federal regulations for patient care and services (CMS, HIPAA, OSHA, and other governing agencies)
  • Participate in quality assurance and cost containment programs

Dialysis Care Center (DCC), a rapidly growing, boutique-style dialysis provider is seeking Peritoneal Dialysis Nurses in the South Suburbs of Chicago, Illinois. Positions are available at our Frankfort, Beverly and Homer Glen locations. We offer on-the-job training for the right candidates.

Position Details: The PD RN will be responsible for training home patients and their families on the safe and effective operation and maintenance of all Home Program specific equipment through a formal Home Dialysis Training program.

The responsibilities of this position include, but are not limited to:

  • Provides safe and effective training, direct patient care and delivery of home dialysis therapies to patients in compliance with standards outlined in the facility policy and procedure manuals, as well as regulations set forth by the state and federal agencies.
  • Rotates with other licensed staff/Home Caregivers to ensure adequate patient care management coverage.
  • Participates in staff training and orientation of new staff as assigned.
  • Participates in required staff meetings as scheduled.
  • Assists in the identification and evaluation and selection of candidates for the HDS Home Dialysis Program, including assessment of patients’ home environment.
  • Trains home patients and their families on the safe and effective operation and maintenance of all Home Program specific equipment through a formal Home Dialysis Training program.
  • Educates patients and family regarding End Stage Renal Disease (ESRD) and information specific to all Treatment Options available to patients with ESRD.
  • Assesses patient and family readiness and ability to perform dialysis treatments at home and patients’ home environment for suitability in the ongoing delivery of home dialysis therapy.
  • Provides ongoing education to patients regarding their home dialysis, peritoneal or vascular access and home dialysis therapy, medications, and other related health conditions.
  • Provides oversight and supervision of all Patient Care Technicians/Staff Assistant Caregivers.
  • Provides safe and effective delivery of home dialysis treatment and support care to Home Dialysis Patients as well as Staff Assisted Home patients.
  • Assesses patients’ response to home dialysis treatment by following predetermined protocols.
  • Communicates patient related issues to the Home Program Coordinator, and the patient’s physician as needed.
  • Reports adverse patient events at home or in the facility/clinic as well as equipment and technology related problems to the Home Program Coordinator and physician, including the correct documentation of such events.
  • Assists with the coordination of a required transfer of the assigned patient to the hemodialysis clinic when appropriate.
  • Reviews, transcribes, and enters physician lab orders accurately into the Medical Information System as required.
  • Maintains appropriate skill level in the operation of all home dialysis therapy-related equipment, including the ability to troubleshoot equipment problems with the patient via telephone.
  • Other duties, as assigned

Position Details: The Home Therapy Nurse will be responsible for training home patients and their families on the safe and effective operation and maintenance of all Home Program specific equipment through a formal Home Dialysis Training program.
